I applied to the program last year and was not accepted. Do I still need to submit a "new" application to be considered for this year's class? |
|
YES. Students who have previously applied to one or more of the health professions programs are required to submit a new application. In addition, this application must be submitted prior to the program's application deadline and include updated transcripts from all colleges and universities attended. Transcripts submitted in prior years can be re-used if the applicant has not completed additional courses at that institution. Please read application instructions further related to this issue. |

Do I have to complete the IUPUI application prior to completing the Health Professions Programs application? |
|
YES. You must apply to Indiana University - Purdue University Indianapolis (IUPUI) as a degree seeking student before you apply to the program. All required materials including official transcripts of your current institution must be submitted to the IUPUI Office of Admissions. If you submit your program application before your IUPUI application, your program application will be held in the Health Professions Programs Office and it will not be forwarded to the program admissions committee until you have completed your IUPUI application. NOTE: Please be aware that there is a difference between a degree seeking application and a non-degree seeking application. If you have any questions regarding a previous application that you have submitted to any campus of Indiana University, please call the IUPUI Office of Admissions at (317) 274-4591 or visit their web site at http://enroll.iupui.edu. They can determine if you are required to submit another application. |
| I have previously applied as degree seeking undergraduate to one of the regional campuses of Indiana University, but never attended. Do I still need to complete an application to the IUPUI campus? |
| YES. Although you may not be required to submit another application fee, you are required to submit a new undergraduate application to the IUPUI Office of Admissions and attach "official" transcript(s) from all colleges or universities that you attended. |
| I have already completed a baccalaureate degree from Indiana University. Do I still need to submit an undergraduate application? |
| YES. For tracking purposes, the IUPUI campus requires all students who have completed a baccalaureate degree within the IU system to complete an undergraduate application. Students are not charged an additional application fee. You will only be required to submit transcripts if you have attended another institution after graduating from IU. Will a transcript that says "issued to student" or something equivalent be accepted? |
|
YES. The Health Professions Program application does not require an "official" sealed copy of your transcript If applicable, ask your institution for a "student" copy or "unofficial" copy. However, the program's admissions committee reserves the right to request an official copy of your transcript. Please remember the IUPUI application requires official transcripts of your current and previous institutions be submitted to the IUPUI Office of Admissions. |

When I made a copy of my transcript, the background indicates that it is "void." Will this still be accepted? |
|
YES. Transcripts are normally issued on special paper that is designed to show this type of pattern when they are photocopied. We accept all photocopies of transcripts as long as they are legible. However, the program's admissions committee reserves the right to request an official copy of your transcript. |

I don't have a copy of my transcript, but I have a copy of the semester grade card that I was sent after I completed the semester. Will this be accepted as the equivalent of a transcript? |

NO. A semester grade card may not include all courses taken by the applicant from the college or university. Applicants submitting a semester grade card in place of a transcript will delay the processing of their application packet. Applications considered incomplete at the program's application deadline because of a missing transcript may not be considered with all the on-time completed applications. |

If I have attended any of the Indiana University campuses, do I have to submit a transcript? |
|
MAYBE. If you have ONLY attended Indiana University, you do not need to submit a transcript. However, if you have transfer credit from another institution, you will need to submit a transcript from that institution. Transfer credit from other institutions being listed on the IU record is not sufficient (see next question). |

Since my current college or university has already transferred the credits from my previous institution onto their transcript, do I need to submit separate transcripts from each school? |
|
YES. Separate transcripts from each school must be submitted (see above exception for IU transcripts). We are unable to accept a previous institution's evaluation of your credits from another college or university. In many cases the courses are listed in alphabetical order and do not show the grade received in each course. |

I have only taken one course from a particular college or university. Since that course is not a program prerequisite, do I still need to submit a transcript? |
|
YES. The admissions committee of each program considers all courses taken at the university level, not just prerequisite courses. Applications considered incomplete at the program's application deadline due to a missing transcript may not be considered with all the on-time completed applications. |

When I was in high school, I received college credit for a course taken during my senior year. Do I still need to send a transcript from this institution? |
|
YES. Regardless of when you took the course, a copy of the transcript is required. Applicants failing to submit any transcript will delay the processing of their application packet. Applications considered incomplete at the program's application deadline due to a missing transcript may not be considered with all the on-time completed applications. |
