Students seeking admission to Health Professions Programs must complete an application for enrollment on the IUPUI campus http://www.iupui.edu/apply or be currently enrolled.  In addition to being admitted to IUPUI, students must meet the following criteria:

   Medical Imaging Technology, BS	→     November 15, 2009 (Entry Summer Session II 2010)
   Nuclear Medicine Technology, BS	→     November 15, 2009 (Entry Summer Session II 2010)
   Paramedic Science, AS	→     October 1, 2009 (Entry Spring 2010)
   	→     February 1, 2010 (Entry Fall 2010)
   Radiation Therapy, BS	→     December 1, 2009 (Entry Summer Session II 2010)
   Radiography, AS	→     November 15, 2009 (Entry Summer Session II 2010)
   Respiratory Therapy, BS	→     January 1, 2010 (Entry Fall 2010)

2. Criminal History Disclosure:  If applicable follow instructions on application.  Do not indicate any previous criminal history anywhere on the application packet.
3. Previous Applicant:  If I answered "yes," did I indicate the year of intended entry?  Did I submit updated transcripts (if applicable)?
4. Provide Information:  Did I type in all necessary information?  Did I include up to date contact information (address/phone number)?
5. Educational Background:  Did I list all institutions attended after graduation from high school or completion of GED?  Did I submit a copy of each transcript, which is required even if the credits are already posted on my IU transcript?  Applicants to the Radiography Program who have completed less than one year of coursework at the university level must submit a copy of their high school transcript (see page 2 of the HPP Application Instructions for more details).
6. Prerequisite Course Completion Plan:  Did I indicate 'yes' but am still completing some prerequisite courses?  If I answered 'no,' did I list all remaining prerequisite courses in the appropriate terms?
7. Admission Policies:  Have I repeated any courses?  Did I indicate them on the form under repeated courses?  Did I have a semster or semesters where my grades might inhibit my chances?  If so, did I ask about using the fresh start or academic bankruptcy policies?
8. Program Specific Requirements:  Did I attach the certification, questionnaire, or observation form as required by my program of interest?
9. Certification:  Did I sign and date the application as required on page 6?  Did I include my full name and university ID number on all other pages?
